New Patients

Register as New Patient

Patients’ permanent address must be within the practice boundary. Proof of residence, dated in the last 3 months, is required. ID will also be required, in the form of a passport or ID card, if registering on the NHS for the first time.

Patients are registered with a specific doctor but can see a doctor of their choice.

Please print off and complete a Registration Form and Patient Questionnaire for each person in your household who wishes to be registered at our Practice and note that there is a different Questionnaire for under 16s.


Temporary Patient Registrations

Green Meadows Surgery will accept Temporary Residents for up to 15 days. If there are no appointment available we will direct patients to local NHS Walk In Centres, details as below:

In an emergency situation the Green Meadows Surgery doctors will see elderly people or young children/babies staying in our area as a temporary patient but this would only be for up to 15 days and may only occur once. After this time, the patient would need to register as a regular patient.


Students returning home from university/college for their holidays will now be asked if they see their university doctor regularly or would they rather be registered with Green Meadows Surgery. Green Meadows Surgery will not be able to see them continuously when they are home from university for their medication/referrals as we have done in the past as temporary patients.

They would therefore need to ensure that they arrange their medication etc. with their registered doctor at university and not leave it until they get home, which is what happens a lot of the time.

If they do see their university doctor regularly and wish to remain registered there, they can only be seen by us in an emergency situation and not for things such as a sore throat or a cough etc. For these they would need to go to a walk-in clinic as detailed above.

The partners have made the decision to change their temporary resident policy in line with most other surgeries around the country.

Appointment availability is one of the biggest issues our patients have with the Green Meadows Surgery, so by reducing the amount of temporary patients we see we are able to release more of the appointments for our regular patients.

As we have said these are guidelines only, Green Meadows Surgery doctors will always see a patient in an emergency situation and will continue to look after our special needs patients when they are home for the holidays.

Patients from outside our area requiring repeat medication

Patients who have forgotten or run out of their medication need to contact their own GP Surgery and request that they fax their prescription to one of our local pharmacies or arrange for it to be sent electronically (EPS). Green Meadows Surgery can give them local pharmacy details and fax numbers if required.

Disabled Patient Facilities

At Green Meadows Surgery we have tried to take into account the needs of patients with disabilities.  We have wide doors for wheelchair access and provide a disabled toilet in both of our buildings.  All clinic rooms are on ground floor level.
